Here, as you provide your phone number, you will receive your username and password via an SMS. Put your details and you will get an application ID. You have to take print of this, and now you have to login with your username and password by clicking on the link http://eci-citizenservices.nic.in. Take out the print of the page that is display. Now attach your copy of identity proof, address proof, photograph and send the form along with these to the Electoral Officer.
