If you do not want to opt for online application then, you can visit the nearest office and get form number 6A from there. After filling all the details in the form you need to submit the form along with the required documents. Online Application: Everybody is using internet these days and with the help of internet you can also apply your voter card online. How to submit the form
After filling your form you need to paste your colored passport sized photograph on the form. Keep it in mind that the background of the photograph should be black and white. You need to fill the form with your full name, residential address and other information as specified in the form. You must check your form twice before submitting it. You can either apply:

- Via visiting the office
If you are applying via post then, make sure that you will provide a set of photocopies of all your documents. Also send a copy of either pass book or passport for Identification. Keep it in mind that you must provide attested copies.
